Ring entrapment is a common emergency department challenge, particularly when swelling, injury, or compromised circulation necessitates rapid ring removal. Clinicians may also need to remove rings before surgical procedures, imaging studies, or other interventions, and this process can be cumbersome and time-consuming when patients wear rings made of modern hard metals. As materials such as tungsten carbide, titanium, and stainless steel have become increasingly popular in jewelry due to their durability, traditional ring-cutting methods may no longer be sufficient, creating new challenges for healthcare providers tasked with safely and efficiently removing these rings. Study Puts Four Ring Cutters to the Test

A recent study published in the Canadian Journal of Emergency Medicine evaluated the effectiveness of four purpose-designed ring removal devices against these increasingly durable materials.

The study, led by Jeffrey J. Perry and colleagues at Ottawa Hospital, compared the performance of the Dolphin Ring Cutter®, GEM-II Ring Cutter, Raptor Shears, and a manual stainless-steel ring cutter. Twenty-one emergency care professionals completed 242 ring-cutting attempts across tungsten carbide, titanium, and stainless-steel rings. Researchers measured successful ring removal, cutting time, ease of use, and user confidence.

Dolphin Ring Cutter Only Device With A 100% Success Rate

One key finding was that the Dolphin Ring Cutter was the only device to successfully cut all tested ring materials, achieving a 100% success rate. In comparison, the GEM-II Ring Cutter achieved success rates of 62% for tungsten carbide, 65% for titanium, and 83% for stainless steel. The Raptor Shears demonstrated limited effectiveness, successfully cutting only 25% of the titanium rings and failing to cut all other materials tested. The manual ring cutter was unable to cut any of the rings evaluated.

Researchers also found that the Dolphin Ring Cutter had the fastest average successful cutting time at approximately 251 seconds, or just over four minutes. Participants rated it highest for ease of use and confidence, with average scores of 4.8 out of 5 in both categories.

The study focused exclusively on medical-grade, purpose-designed ring-cutting devices and specifically excluded general-purpose rotary tools because they are not medical devices and may pose injury risks in clinical settings.

The findings underscore an important reality for healthcare providers: as ring materials continue to evolve, having access to reliable, purpose-built ring removal equipment can help clinicians address ring entrapment situations more effectively and efficiently.
