The Growing Role of Heated High-Flow Nasal Cannula Therapy

Evidence Continues to Strengthen the Role of HFNC Therapy in Respiratory Care

Heated high-flow nasal cannula (HFNC) therapy has transformed the management of acute respiratory failure by delivering warmed, humidified oxygen at high flow rates while improving patient comfort and reducing the work of breathing. Once viewed as an alternative to conventional oxygen therapy, HFNC is now supported by robust clinical evidence and has earned prominent recommendations in both the 2022 European Respiratory Society (ERS) Clinical Practice Guidelines and the 2026 American Thoracic Society (ATS) Clinical Practice Guideline.

HFNC provides several physiologic benefits that explain its growing adoption. By delivering heated and humidified gas at high flow rates, HFNC can improve oxygenation, reduce anatomical dead space, generate low levels of positive airway pressure, enhance secretion clearance, and improve patient tolerance compared with conventional oxygen delivery systems. These advantages are particularly important for patients with acute hypoxemic respiratory failure, in whom comfort and adherence can significantly affect outcomes.

    What the Guidelines Recommend

The 2022 ERS Clinical Practice Guidelines marked a major milestone for HFNC therapy. After reviewing the available evidence, the ERS task force issued multiple recommendations supporting the use of HFNC in acute respiratory failure. The guidelines suggest HFNC over conventional oxygen therapy for patients with hypoxemic acute respiratory failure and even suggest HFNC over noninvasive ventilation (NIV) in certain hypoxemic patients. Additional recommendations support the use of HFNC during NIV breaks and in selected post-extubation and postoperative populations. The ERS concluded that HFNC is a valuable intervention that can help clinicians select the most appropriate noninvasive respiratory support strategy across a variety of acute care settings.

Building on this foundation, the 2026 ATS Clinical Practice Guideline further elevated the status of HFNC. Most notably, the ATS issued a strong recommendation for HFNC over standard oxygen therapy in adults with de novo acute hypoxemic respiratory failure, based on moderate-certainty evidence. The guideline also recommends HFNC or NIV for preoxygenation before endotracheal intubation and suggests HFNC following extubation in critically ill patients. Additionally, ATS recommends HFNC as an alternative to NIV in carefully selected patients with acute hypercapnic respiratory failure and mild acidemia when close monitoring and prompt escalation are available.

The evidence supporting HFNC continues to grow. According to the ATS guideline, pooled data from randomized clinical trials demonstrated that HFNC reduced the need for intubation among patients with acute hypoxemic respiratory failure compared with standard oxygen therapy. The guideline panel determined that the benefits of HFNC clearly outweighed potential harms, helping establish HFNC as a frontline respiratory support modality in modern acute care practice.

A Complete High-Flow Solution: The Invent Respiratory HFT150®

As clinical adoption increases, healthcare providers need high-flow systems that combine evidence-based performance with operational simplicity. This is where the Invent Respiratory HFT150® Heated High-Flow System offers a unique solution.

The HFT150 is a fully integrated heated high-flow therapy platform designed to deliver warmed, humidified oxygen and air at flow rates from 2 to 60 LPM. Unlike systems that require multiple external components, the HFT150 integrates an air/O₂ blender, heated humidifier, internal battery, nurse-call alarm, and optional SpO₂ monitoring into a single platform. Its Servo Auto Flow (SAF) technology helps maintain precise flow delivery even when leaks or varying back pressures are present. In contrast, independent control of flow, temperature, and FiO₂ allows clinicians to tailor therapy to individual patient needs.

    Enhancing Comfort with the HFC-GOLD®

Complementing the HFT150 is the HFC-GOLD® High-Flow Nasal Cannula. Designed with a soft, contoured interface, curved nasal prongs, and ergonomic headgear, the HFC-GOLD prioritizes patient comfort and a secure fit. Multiple adjustment points help reduce pressure points while maintaining consistent therapy delivery, even in challenging patient positions. The cannula’s compatibility with a wide range of respiratory devices further enhances its versatility in busy clinical environments.

    Aligning Technology with Evidence-Based Care

As the ATS and ERS guidelines continue to validate the clinical value of heated high-flow therapy, providers are increasingly looking for solutions that support both excellent patient outcomes and efficient workflow. Together, the Invent Respiratory HFT150® Heated High-Flow System and HFC-GOLD® High-Flow Nasal Cannula provide a comprehensive high-flow therapy platform that aligns with today’s evidence-based respiratory care standards while prioritizing comfort, performance, and ease of use.
